How to order exams and materials:
Orders for exams and training materials can be submitted via email, fax, phone and mail. Email orders to examorders@Prometric.com., fax to 800.347.9724, phone to 800.624.2736 or mail to Prometric, Food Safety, 1260 Energy Lane, St. Paul, MN 55108. Please include the Materials and Exam Order Form and your payment information. Training materials and all shipping and handling must be paid for at time of order. Payment Options:
If you want to be invoiced, we will accept a purchase order (PO) provided that you are submitting more than ten exams to be scored. All exams are paid for at time of scoring not at the time they ordered. A hard copy of the PO must accompany the order. No PO numbers will be taken over the phone. The PO can be emailed, phoned, faxed or mailed to the numbers and address listed above. The PO must have the company name/logo, billing address and phone number, quantity of items being ordered and/or number of exams being sent in for scoring. A separate PO must accompany each training materials order or batch of exams to be scored. No "open" POs will be accepted or kept on file.

If you are submitting fewer than 10 exams you must include your payment with a completed materials and exam order form. We accept Visa, MasterCard, American Express, money order, cashier's check or company check for any order quantity.
 
Quantity of exams to order:
Please order the number that you know you will use. We understand that at times you order extra to accommodate last-minute candidates.  Exams change every three months; however, they are valid for a year.  If you find you have too many exams, they can be used with your next class.  Please be sure to use your oldest exams first.  There are no additional fees or charges for returning unused exams.
